    I accept DECLARATION.

    DECLARATION : -

    I hereby declare that the above statement are true to the best of my knowledge and belief and the I am free from the disqualification menontied in the Delhi State Government Paramedical Council Act, 2008, rules and regulations. I promise in the event of being registered and in consideration thereof to be bound by and to conform in all respects to the rules, regulation etc. framed by Council form me to me in force.


    Instructions for Filling Registration Form

    1. Applicant shall enclose a D.D. of Rs. 8,500/- (for each course, Delhi Residents Only) drawn in favor of Registrar, Delhi State Government Paramedical Council. Applicant registered with other State Councils/ Institutions/ Colleges/ Universities and Other Educational Institutions will pay 10,500/- (Not A Delhi Resident) as registration fee and applicant qualified from other countries will pay Rs. 18,500/- as registration fee.

    2. Registration fee will not be refunded for any reason.

    3. Fasttrack Mode: quick application can be done depending on students situation and requirements for which there is a verification fees (vary as per institute/ organisation) and distance fees for our field officer to visit and get the things done on priority basis which will cost Rs 25/KM from our place to center.

    4. Applicant applied Good Standing Certificate charges Rs 2,500/- with Registration, otherwise Rs 4,000/- will be the charges after getting Registration Certificate. Our Registration Validity is for 10 Years from the date of issue.

    5. The following documents should be enclosed with the duly filled application form in the order below.

    1. Demand-draft in favor of Registrar, Delhi State Government Paramedical Council, Delhi.
    2. Attested copy of the letter of the recognition of the institution/college/university (issuing diploma/degree) from any State Government or Central Government for the purpose. (2 copies both sides, if any).
    3. Self Attested copy of Secondary School Marks Sheet/Certificate to prove the date of birth. (2 copies both sides).
    4. Self Attested copy of 10+2 mark Sheet/Certificate (2 copies both sides).
    5. Self Attested copy of Certificate of qualifying examination for which registration is sought (2 copies both sides).
    6. Self Attested copy of Cast Certificate and Bona-fides Certificate. (2 copies both sides).
    7. Self Attested copy of Aadhar Card (2 copies)

    Note:

    • It is mandatory for the student to get the registration done within three years after completing the course.
    • If any student fails to get the registration done within three years then five hundred rupees yearly panelty will added from the date of course completion.
